Size is a combination of the Growth/Shrink power (size from -2 to 11+) and the Size, Toughness Modifers Table (size from -2 to 10). House rule: instead of Size, Toughness Modifers Table, simply add the creatures Size to it's Toughness and apply the Minimum Toughness rule after all other adjustments.
Category comes from the Growth/Shrink power (for Large, Huge and Gargantuan). Small comes from Size Attack Modifiers. Size -1 is Medium so humans with the Small hindrance would not have attack modifiers.
Attack comes from Size Attack Modifiers. Attackers add the target's modifier and subtract their modifier from attacks. Examples: Cat attacking rhino modifier is 4 = (2 -(-2)). Dragon attacking human modifier is -4 = (0 - 4).
Inches comes from the Growth/Shrink power.
